All accepted students must submit the a variety of financial and medical documents prior to move in. All accepted students will have a private intake meeting prior to their start typically held between June-July. At this time, all documents must be submitted. All accepted students will receive a packet in the mail outlining all the required forms which must be completed by the in-take date. All students must submit a photo copy of the following documents:
- Social Security Card
- Birth Certificate
- Medicaid Card and/or Medicare Card (Medicaid Wavier applicants only)
- Health Insurance Card
- Guardianship Papers, if applicable
- Information on all active Trust’s (Medicaid Waiver applicants only)
- Photo ID card
